The noxious draconic visage is an award obtained from the Wise Old Man in exchange for Velgar's head, which is obtained by killing Velgar during or after the quest Dragon Slayer. It can be used to upgrade the anti-dragon shield into the dragonblight shield.

Trivia
- The visage appearance resembles that of an Wyvern and Skeletal visage.
- Unlike Old School RuneScape and RuneScape, the visage is not dropped by Velgar, it is instead handed to the players as a reward. Green dragons do not drop these visages also.
